The Illinois State Police has reported and released the information on a fatal crash on I-57 just north of Salem at milepost 117 Wednesday night.

Marion County Assistant Coroner Tina Spradin pronounced 20-year-old Shane Michael Donegan of Warren, Michigan dead at the scene after a van he was riding in slammed into a parked semi.

Investigative details from the ISP say the van was in the northbound lane, driven by 22-year-old Jesse L. Meacham, also of Michigan, when it ran off the right side of the road and struck the semi with its driver, 56-year-old Dennis Fuller of Phoenix, Arizona inside.

Meacham was taken to Salem Township Hospital and then flown to a St. Louis area hospital for treatment of injuries while Fuller refused treatment.

Meacham was cited for improper lane usage. Fuller was ticketed for driving while disqualified, on an expired medical card, parking where prohibited, and disobeying controlled access highway provisions.

The crash is still under investigation.
