A Marion skilled care facility has been fined twice by the Illinois Department of Public Health for violating the Nursing Home Care Act during the fourth quarter of last year.
Officials say Parkway Manor, a 131-bed skilled care facility, was fined $25,000 for an “A” violation in October and fined $25,000 for the same violation in December.
The violation pertains to a condition in which there is a substantial probability that death or serious mental or physical harm will result, or has resulted.
