The Hamilton Memorial Hospital District Foundation is now accepting nominations for the 2017 Distinguished Healthcare Provider of the Year Award.

The award will be presented at the Foundation’s 18th annual Foundation Dinner, set for Saturday, Nov. 11 at Hamilton County Senior High School in McLeansboro.

The award recognizes an individual – living or deceased – or group who has made an outstanding contribution to the health and welfare of residents of our area.

Nomination forms are available on the Hamilton Memorial Hospital website at hmhospital.org under the “News” tab and at the hospital front desk.

Completed nomination forms must be received by Oct. 27.
